Hyperpigmentation vs. Melasma: What’s the Difference?
Brown spots, uneven skin tone, patches of discoloration—it's easy to look at pigmentation and assume it's all the same thing.
But hyperpigmentation and melasma are not exactly the same, and understanding the difference is important because they may require very different treatment approaches.
At The Derm Lounge, we treat both hyperpigmentation and melasma, but how we approach each depends on what is causing the pigment, how it presents in the skin, and how it responds to treatment.
What Is Hyperpigmentation?
Hyperpigmentation is an umbrella term used to describe areas of the skin that become darker than the surrounding skin due to increased melanin production.
It can be triggered by several different factors, including:
- Sun exposure and accumulated sun damage
- Acne and inflammation
- Skin injuries
- Hormonal changes
- Certain medications
- The natural aging process
Hyperpigmentation can appear as individual brown spots, clusters of discoloration, post-acne marks, or areas of uneven skin tone.
Think of hyperpigmentation as the larger category. There are multiple types and causes of excess pigment—and melasma happens to be one of them.
So, What Is Melasma?
Melasma is a specific type of hyperpigmentation**, but it tends to behave differently from the sun spots and post-inflammatory pigmentation we commonly see.
Melasma often appears as larger, symmetrical patches of brown or gray-brown pigmentation, particularly across the:
- Cheeks
- Forehead
- Upper lip
- Nose
- Chin
One of the biggest differences is what drives it.
While traditional hyperpigmentation may develop following sun exposure, acne, or inflammation, melasma is often influenced by a combination of hormones, genetics, UV exposure, visible light, and heat.
This is one reason melasma can be particularly stubborn. Even after improvement, it can return when triggered again.
Why Does the Difference Matter?
Because when it comes to pigmentation, more aggressive doesn't always mean better.
Certain types of hyperpigmentation can respond beautifully to treatments that specifically target unwanted pigment.
Melasma requires a more thoughtful approach. Because the pigment-producing cells are easily stimulated, excessive heat, inflammation, or overly aggressive treatment can potentially trigger more pigmentation.
That's why properly identifying the type of pigmentation we're working with is an important first step.
How We Treat Hyperpigmentation
At The Derm Lounge, there isn't one standard protocol for every patient with pigmentation.
Depending on your skin and the type of hyperpigmentation present, we may incorporate treatments such as:
Mesoneedling + Customized Polymicronutrients (from Toskani)
Mesoneedling allows us to work with customized solutions selected specifically for the patient's skin concerns. For pigmentation, we can incorporate polymicronutrient cocktails designed to support brighter, more even-looking skin while addressing overall skin quality.
IPL (Intense Pulsed Light)
IPL can be an excellent option for appropriate patients with visible sun damage, brown spots, and uneven pigmentation. Light energy targets areas of excess pigment, which can gradually become less noticeable as the skin renews.
MOXI
MOXI is a gentle fractional laser treatment that can be used to improve uneven pigmentation, sun damage, tone, and overall skin quality.
Depending on the patient, these treatments may be used individually or incorporated into a larger treatment plan.
Chemical Peels
Chemical Peels can also be helpful, depending on the type of peel!
How We Approach Melasma
Melasma is different—and we treat it that way.
Rather than simply trying to eliminate every visible area of pigment as aggressively as possible, the goal is to improve the appearance of melasma without unnecessarily stimulating the skin.
At The Derm Lounge, MOXI is one of our preferred treatment options for appropriate patients with melasma.
Because melasma is a chronic, trigger-sensitive condition, treatment is typically about management rather than a permanent "cure."
Sun protection and proper at-home skincare are also incredibly important. Even after achieving beautiful improvement, UV exposure, visible light, heat, and hormonal changes may cause melasma to become more noticeable again.
Hyperpigmentation vs. Melasma at a Glance
Hyperpigmentation is the broad term for excess pigment or discoloration in the skin. It can be caused by sun damage, acne, inflammation, injury, hormones, and other factors. Depending on the patient, we may treat it with mesoneedling + customized polymicronutrients, IPL, MOXI, or a combination approach.
Melasma is a specific type of hyperpigmentation that is often influenced by hormones, genetics, UV exposure, visible light, and heat. It commonly appears in symmetrical patches on the face and tends to be more persistent and prone to recurrence. For appropriate patients, MOXI may be incorporated into a customized treatment plan.
